Orders by Fax
To place an order by fax, please use our Order by Fax form provided in Acrobat Reader format or Ordenes por el fax, for Spanish speaking Americans. Fill in all the information required, and fax it to 514.345.0123, 24 hours a day. If you choose to do payment by credit card, please include all information necessary. If you choose to do payment by check, fax a copy of the check along with the order form, and mail both the order form and the original check to one of the following addresses.
